SUNSCREEN PERMISSION

It's getting warmer! During hot and sunny days we ask that your child be covered in sunscreen at the time of drop-off. By submitting the form below you authorize ChildRoots to apply either parent provided sunscreen, or ChildRoots provided SPF Rx broad spectrum mineral sunscreen with an SPF 50 with an active ingredient of Zinc Oxide to exposed areas - e.g., face, arms, etc. - in the afternoon.
